I don't know the answer to that last question of yours. I have
wondered it myself in relation to online annotation of video
(quicktime, flash, realMedia, anything that streams).
Recently I picked up an O'Reilly book called "Quicktime for Java"
that I hope will give me a few clues. I may have to finally learn
Java for real. Or maybe it is something more straightforward...

yes you can. There was a custom QT client written by a design firm in
Sweden which allowed live chat over a live stream all done in QT. QT
reads XML files, and you can modify behaviour, or add text layers, on
the basis of the xml. obviously the xml can be generated on any basis
you wish.
Matthew Peterson's book on Interactive QuickTime has a very useful
chapter on this.
