yes you can. There was a custom QT client written by a design firm in 
Sweden which allowed live chat over a live stream all done in QT. QT 
reads XML files, and you can modify behaviour, or add text layers, on 
the basis of the xml. obviously the xml can be generated on any basis 
you wish.

Matthew Peterson's book on Interactive QuickTime has a very useful 
chapter on this.
